A 41-YOW with alcoholism had alopecia, diarrhea, angular cheilitis & pruritic eruption

หญิงอายุ 41 ปีมารพ.เนื่องจากมีผมร่วงและผมงอกเป็นเส้นอ่อนๆ ท้องเสีย อักเสบที่มุมปาก มีผื่นแดงตกสะเก็ดและคันที่มือเท้าส่วนปลาย ที่perineum และที่ก้นมานาน 5 เดือน ผู้ป่วยติดสุราเรื้อรัง มีความดันโลหิตสูงและ type 1 diabetes mellitus ซึ่งมีภาวะแทรกซ้อนคือ retinopathy, nephropathy และ end-stage renal disease. เธอไม่มีอาการทางระบบประสาทใดๆ ผู้ป่วยได้รับการรักษาเรื่องผื่นด้วย topical corticosteroids และ antifungal agents และด้วยยาปฏิชีวนะรูปกินแต่อาการไม่ดีขึ้น

Acquired Acrodermatitis Enteropathica

- rare skin condition

- first described in patients with zinc deficiency.

- primary form is autosomal recessive. However, similar appearing lesions have been described in patients with amino acid deficiencies.

- Classically , the patients present with erythematous patches and plaques of dry, scaly, eczematous skin that may evolve into crusted, vesiculobullous, erosive and pustular lesions.
- The lesions are distributed in a periorificial and acral pattern, on the face, scalp, hands, feet, and anogenital areas.
- Paronychia as well as loss of scalp hair, eyebrows and eyelashes may occur.
- Secondary infection with Staphylococcus aureus and Candida albicans may occur.
- When this disease occurs in infants, symptoms such as withdrawal, photophobia and loss of appetite may occur.
- Abnormalities in the epithelium of the gut often leads to diarrhea.

- sent ZINC level from diagnosis and other metabolic due to comorbidity
- Treatment with zinc supplementation produces rapid improvement.

สิ่งที่ครูต้องการให้ลูกศิษย์ได้ไป ไม่ใช่ต้องจดจำทุกอย่าง (ขอให้จำเฉพาะที่สำคัญและเป็น core) แต่ต้องสามารถค้นหาคำตอบได้เอง

การวินิจฉัย: Late acquired acrodermatitis enteropathica และยืนยันด้วยการตรวจพบว่า serum zinc levels ต่ำมาก คือ 0.35 µg per milliliter (ปกติ range, 0.66 - 1.10 µg per milliliter)

ผู้ป่วยได้รับการรักษาด้วย oral zinc sulfate supplementation และภายใน
3 สัปดาห์หลังเริ่มยา อาการท้องเสียหายไป ผมเริ่มงอกขึ้นใหม่และผื่นหายไป
