A 79-YOW had induration at right leg 3 months after undergoing acupuncture

หญิงอายุ 79 ปี มีก้อนแข็ง (induration) ที่ขาขวามานาน 3 เดือนหลังจากไปฝังเข็มเพื่อรักษาเข่าเสื่อมทั้ง 2 ข้าง induration นี้อยู่รอบจุดฝังเข็มซึ่งอยู่ในแนว gallbladder meridian (site GB 38, or Yangfu) (รูป 1A).

1) รูป 1B และ 1 C เป็นภาพรังสีของ right leg พบความผิดปกติอะไรบ้าง
2) การวินิจฉัยที่น่าจะเป็นคือ......................................
3) จะ manage อย่างไรต่อ

ขอตอบค่ะ
ข้อมูลที่สำคัญ
1. ผู้ป่วยมีก้อนแข็ง (induration) ที่ขาขวามานาน 3 เดือนหลังจากไปฝังเข็ม
2. จากภาพรังสีของขา พบมี calcified spot ตรงตำแหน่งที่ฝังเข็ม และ periosteal bone formation (ตามแนวหัวลูกศร)

คิดว่าผู้ป่วยมี chronic soft tissue and bone infection ที่เกิดจากการฝังเข็มนำเชื้อเข้าไป เชื้อก่อโรคเรื้อรังก็นึกถึง TB และ NTM, fungi, nocardia,และ melioidosis ค่ะ

ก็ต้องเจาะเอาเนื้อเยื่อที่ก้อนมาย้อมสีแกรม, acid fast +modified acid fast และส่ง culture for aerobic bacteria, mycobacteria และ fungus ค่ะ

Radiographs of the right leg showed focal dystrophic calcification below this point. The underlying fibula was intact except for periosteal bone formation along the lower half (รูป B และ C). The findings were consistent with the presence of a chronic soft-tissue infection that extended along the muscle compartments over the fibula.

ไม่พบอาการแสดงของ trauma หรือตำแหน่งของการติดเชื้ออื่นๆ.
ภาพรังสีทรวงอกปกติ

Histologic studies of the biopsy specimen showed suppurative granulomatous inflammation with microabscesses, caseous necrosis, and acid-fast bacilli.

ผลเพาะเชื้อของ biopsy specimen ขึ้น Mycobacterium chelonae.

ผู้ป่วยได้รับการรักษาด้วย imipenem, clarithromycin และ amikacin นาน 3 สัปดาห์ตามด้วย clarithromycin และ minocycline นาน 6 เดือน ผู้ป่วยหายดีหลังจากนั้น
