An 88-YOM presented with diffuse, painless swelling of lateral borders of tongue




   ชายอายุ 88 ปีมารพ.เนื่องจากมีอาการบวมที่ขอบลิ้นทั่วๆไป (รูป A) อาการบวมนี้เริ่มเป็นมานาน 20 ปี.
ผู้ป่วยไม่มีโรคประจำตัวใดๆ, ไม่ดื่มสุรา และไม่มีสมาชิกในครอบครัวเป็นเหมือนผู้ป่วย

PE: Yellowish tumors with a smooth, shiny surface were visible. There was no frank macroglossia, dysphagia, dysarthria, or sleep apnea. No other tumors were identified anywhere in the body, including the oral cavity and the gastrointestinal tract, which were evaluated endoscopically. There was no cervical lymphadenopathy.

-->1.การวินิจฉัยที่น่าจะเป็นคือ............
พยาธิสภาพที่เห็นคือ smooth, well-circumscribed and round-to-oval mass ซึ่งสามารถ differential diagnosis ได้คือ infiltrating lipoma ซึ่งต้องวินิจแยกโรค malignant liposarcoma ออกไปก่อน, benign lipomatous tumour เช่น benign lipoblastomatosis, hibernoma, intramuscular myxoma หรือ infiltrating angiolipoma.
-->2. จะ manage อย่างไร
1.การวินิจฉัยต้องอาศัย histopathologic examination คงต้องส่ง specimen หลัง surgical excision
2.การรักษาคือ surgery ซึ่งสิ่งที่พยากรณ์โรคที่ดีคือ histology อ่านเป็น free of tumour magin แม้ว่าได้รับการผ่าตัดพบว่ามี recurrence rate ได้ถึง 3-62.5 % และเวลาการ recurrence เท่ากับ 6 เดือนถึง 20 ปี

ผู้ป่วยได้รับการทำ incisional biopsy ที่ลิ้น ในรูป B เป็น histology ของรอยโรคที่ลิ้น

histology ของรอยโรคที่ลิ้น: lobules of mature adipose tissue that were consistent with lipomatosis (arrows); there was no evidence of liposarcoma.

Serum glucose และ lipid levels ปกติ
ผู้ป่วยไม่ได้รับการรักษาใดๆ
isolated lipomatosis of the tongue พบได้น้อย
