A 72-YOW had progressive whitening of her nails for 4 months

หญิงอายุ 72 ปีซึ่งมีโรคเดิมคือ diabetes mellitus, hypertension และ peripheral vascular disease ได้เข้ารับการรักษาในรพ.เนื่องจากเจ็บหน้าอกและหอบเหนื่อย เธอมีอาการเหนื่อยง่ายเวลาออกแรง นอนราบไม่ได้ และเล็บมือและเท้าเปลี่ยนเป็นสีขาวมากขึ้นมานาน 4 เดือน

Physical examination revealed an elevated jugular venous pressure, an S4 gallop rhythm, bilateral pulmonary rales, peripheral pitting edema, and complete whitening of the nail units of her hands and feet.

1. ผู้ป่วยมีปัญหาอะไรบ้าง
2. เล็บขาวในผู้ป่วยรายนี้เกิดจากอะไร

-ผู้ป่วยอายุ 72 ปี
Problems
1 Chest pain and dyspnea with signs of congestive heart failure (4 mo.)
2. White nails+toes (4 mo.)
3. diabetes mellitus,
4. hypertension
5. peripheral vascular disease

White nails เป็นมาพร้อมกับอาการของ CHF ไม่ทราบว่า CHF จะมีผลทำให้เกิด white nails ได้หรือไม่
จากในรูป นิ้วของผู้ป่วยส่วนปลายเรียวเล็กลงทุกนิ้ว น่าจะเป็นผลจาก peripheral vascular disease ทำให้เกิด tissue hypoxia คิดว่า peripheral vascular disease อาจมีผลต่อการเกิด white nails ได้ด้วยกลไกเดียวกัน

-->ข้อ2. เล็บขาวในผู้ป่วยรายนี้เกิดจากอะไร
ลักษณะเล็บของผู้ป่วยรายนี้คือ white discoloration of the nail plate หรือเรียกว่า leukonychia ซึ่งอาจแบ่งเป็น true leukonychia และ pseudoleukonychia ในผู้ป่วยรายนี้คิดว่าเป็น true leukonychia เนื่องจากลักษณะเนื้อเล็บขาวทั้งหมดทั้งนี้เกิดจากมีพยาธิสภาพที่ matrix ซึ่งแตกต่างจาก psudoleukonychia ที่เป็นการเปลี่ยนแปลงเพียง nail bed (peudoleukonychia มี 3 subtypes คือ half and half nail, Terry’s nail และ Muehrche’s nails) อีกทั้งในผู้ป่วยรายนี้มีลักษณะเป็น total leukonychia ซึ่งเกิดทั้งใน congenital (ซึ่งถ่ายทอดทางพันธุกรรมแบบ AD และ AR) และ systemic disease ซึ่งในผู้ป่วยรายนี้คิดถึง systemic disease มากกว่า ส่วน condition ที่อาจทำให้เกิด total leukonychia แบบ acquired นี้ ได้แก่ typhoid fever, leprosy,cirrhosis, nail biting, trichinosis and cytotoxic drugs แต่ในรายนี้คิดถึง cardiac failure มากที่สุดเพราะว่าเกิดขึ้นพร้อม ๆ กับเล็บที่ขาวขึ้น

Laboratory investigations: normal complete blood count, as well as normal renal and liver function.
Electrocardiography revealed deep T-wave inversions in leads V1 through V3.
Echocardiography showed an ejection fraction of 40 percent with apical-wall hypokinesis, and coronary angiography demonstrated a 90 percent stenosis of the midportion of the left anterior descending artery, confirming the diagnosis of ischemic cardiomyopathy.

เล็บขาวสามารถเกิดขึ้นโดยเป็นผลของการเปลี่ยนแปลงของ nail plate (true leukonychia) หรือ injury ต่อ nail bed (pseudoleukonychia). Acquired leukonychia totalis มีความสัมพันธ์กับ several systemic diseases, รวมถึง hepatic cirrhosis, chronic renal failure, congestive heart failure, diabetes mellitus, chronic hypoalbuminemia, และ Hodgkin”s lymphoma.

ผู้ป่วยรายนี้ได้รับการทำ coronary angioplasty with stent insertion in the affected artery. จากการติดตามที่ 6 เดือนต่อมา เล็บของเธอกลับเป็นปกติ.
