A 59-YOW had persistent swelling of her arm after injured left elbow




   หญิงอายุ 59 ปีตกลงจากหลังม้าและมีบาดแผลที่ข้อศอกซ้าย 4 วันต่อมาเธอมาพบแพทย์อีกเนื่องจากแขนยังบวมมากอยู่

On examination, the patient had a small, clean wound over the olecranon; there were no signs of infection. Diffuse crepitus of the arm, extending to the left chest wall, was noted.

1. ในรูปคือ oblique radiograph of the elbow (รูป A) และ posteroanterior radiograph of the chest (รูป B) พบความผิดปกติอะไรบ้าง
2. จะ manage ผู้ป่วยอย่างไรต่อไป

   ขอตอบค่ะ

อยากทราบว่านอกเหนือจากเรื่องแขนและหน้าอกบวม (subcutaneous emphysema)ผู้ป่วยมีอาการหอบเหนื่อยหรือไม่ vital signs ดีไหมคะ

จาก CXR และ film แขนซ้าย พบว่ามี subcutaneous emphysema ไม่มี pneumothorax หรือ pneumomediatinum

น่าจะ observe ผู้ป่วยค่ะ ยังไม่ทำอะไร

Oblique radiograph of the elbow (รูป A) และ posteroanterior radiograph of the chest (รูป B) demonstrated circumferential subcutaneous emphysema (arrows) of the upper extremity extending from the wrist to the left hemithorax. No rib fracture or pneumothorax was seen.

การไม่มี air อยู่ภายในกล้ามเนื้อแสดงว่าไม่น่าจะมี deep, gas-producing soft-tissue infection

ในผู้ป่วยรายนี้ subcutaneous emphysema หายไปเองโดยหลังจากนั้น 1 สัปดาห์ โดยไม่ต้องมีการผ่าตัด

สาเหตุของ subcutaneous emphysema ที่ไม่ใช่การติดเชื้อ ได้แก่ perforation of the pulmonary or digestive tracts, blast injuries, cutaneous ulcers, air-gun injuries, dental extraction, และ irrigation of wounds with hydrogen peroxide. ในผู้ป่วยรายนี้ อธิบายกลไกการเกิด subcutaneous emphysema ได้ว่าเกิดจากการที่มีอากาศปริมาณมากเข้าสู่ soft tissues จากบาดแผลเล็กๆ ที่ผิวหนังโดย ball-valve mechanism
