A 69-YOW had intense lingual pain developed before pacemaker implantation

หญิงอายุ 69 ปีซึ่งมี complete atrioventricular block ได้เข้ารับการรักษาในรพ.เพื่อทำ pacemaker implantation ผู้ป่วยมีโรคเดิมคือ multiple sclerosis ซึ่งทำให้เกิด tetraparesis เธอได้กินยา acenocoumarol มานาน 10 ปีแล้วเพื่อรักษา deep vein thrombosis.
12 ชั่วโมงหลังหยุด acenocoumarol (เพื่อเตรียมสำหรับ pacemaker implantation) ผู้ป่วยมีอาการเจ็บที่ลิ้นอย่างรุนแรง

จากการตรวจร่างกาย พบว่าลิ้นมีลักษณะดังในรูป A

1. การวินิจฉัย: Necrosis of tongue
สาเหตุ ผู้ป่วยน่าจะมี underlying disease ที่ทำให้มี hypercoagulable state เมื่อหยุด anticoagulant --> clot --> ทำให้เลือดไปเลี้ยงที่ลิ้นลดลงอย่างมาก --> necrosis
2. ช่วงแรก check CBC +platelet count และ coagulogram ค่ะ

On physical examination, a well-circumscribed, triangular necrotic area was detected on the tip of the tongue (รูป A). ผู้ป่วยได้รับการผ่าตัดตัดลิ้นส่วนที่มี necrosis
ในรูป B คือ histopathology ของลิ้นที่มี necrosis
Histopathological analysis revealed ischemic necrosis and intraluminal thrombi without vasculitis

จาก CBC พบว่า total peripheral-blood platelet count= 700,000/cumm และ ได้ให้การวินิจฉัย essential thrombocytosis ซึ่งมีการยืนยันด้วย bone marrow biopsy และการตรวจพบ JAK2 V617F mutation ไม่เคยทราบค่า platelet count ของผู้ป่วยมาก่อน ได้เริ่มการรักษาด้วย acenocoumarol และ hydroxyurea และผู้ป่วยหายดี

ลิ้นเป็นตำแหน่งที่พบการเกิด ischemic necrosis ได้น้อย แม้กระทั่งในผู้ป่วย essential thrombocytosis เนื่องจาก blood supply และ collateral circulation ดีมาก.
