A 45-YOM with acute leukemia had productive cough after neutropenia




   ชายอายุ 45 ปีได้เข้ารับการรักษาในรพ.เพราะมี relapse ของ acute promyelocytic leukemia ที่ 9 เดือนหลังได้ remission ครั้งแรก
เขามี severe neutropenia เกิดขึ้นหลัง salvage chemotherapy หลังได้รับ mitoxantrone, cytarabine, และ all-trans -retinoic acid
ผู้ป่วยไม่มีไข้และไม่มีหอบเหนื่อย มีแต่ไอ เสมหะสีเหลืองออกน้ำตาล

1. รูป A เป็น chest x-ray film ของผู้ป่วย พบความผิดปกติอะไรบ้าง
2. การวินิจฉัยคือ........................................................................

   ขอตอบค่ะ
ใน CXR พบว่ามี ก้อนที่ RUL field อยู่ใน cavity มีอากาศอยู่รอบๆ เป็น halo sign หรือ air crescent sign.
การวินิจฉัยคือ invasive pulmonary aspergillosis (IPA) มากกว่า aspergilloma เพราะผู้ป่วยเป็น acute leukemia และมี severe neutropenia หลังได้รับ chemotherapy

รูป B คือ CT chest ของผู้ป่วย

CXR revealed an air-crescent sign around the sequestrum, and computed tomography showed that the lesion had invaded the right main bronchus, resulting in the formation of a fistula (ลูกศรสีขาวในรูป B) The lesion was also close to the right pulmonary artery (ลูกศรสีขาวในรูป B), and there was tissue inflammation outside the posterior wall of the cyst that extended through the pulmonary parenchyma (open arrow ในรูป B).

ผู้ป่วยได้รับการรักษาด้วย amphotericin B และได้รับการผ่าตัด right superior pulmonary lobectomy โดยเร็วหลังจากสภาพทางคลินิกดีพอ

Histologic examination of the resected right upper pulmonary lobe showed a typical fungus ball measuring 4 by 4 by 3.5 cm that was surrounded by a pink exudative material that contained many necrotic aspergillus mycelia. The sequestrum communicated with the right main bronchus by means of a fistula.

ผู้ป่วยหายดีหลังจากนั้น จากการติดตามที่ 9 เดือนหลังการผ่าตัด ผู้ป่วยไม่มีอาการทางปอดและ acute leukemia ยังอยู่ใน remissionครั้งที่ 2.
