A 33-YOM presented with a rapidly growing, fungating ulcer in the burn scar




   ชายอายุ 33 ปีซึ่งเคยโดนไฟไหม้ที่ต้นแขนขวาตั้งแต่วัยเด็ก มารพ.เนื่องจากมีแผลที่ลุกลามเร็วที่แผลเป็นจากไฟไหม้

1. ในรูปคือรอยโรคที่ต้นแขนขวาของผู้ป่วย

การวินิจฉัยคือ Marjolin ulcer ค่ะ (malignant ulcer which occurs in area of chronic scarring)

A biopsy of the mass revealed invasive squamous-cell carcinoma consistent with Marjolin”s ulcer.

ผู้ป่วยได้รับการผ่าตัด wide local excision and placement of a split-thickness skin graft. No evidence of tumor was identified in the sentinel lymph nodes.

Dr. Jean Nicolas Marjolin เป็นคนแรกที่บรรยายการเกิด ulcerating lesions บน scar tissue เมื่อปีคศ. 1828. Marjolin”s ulcer is เป็น term สำหรับ aggressive epidermoid tumors ที่เกิดขึ้นบนตำแหน่งที่มี chronic injury โดยพบบ่อยว่าเกิดขึ้นที่ตำแหน่งของ burn wounds.
