A 67-YOM had pain, vesicles, and erythema of the right side of forehead for 3 days

ที่หน้าผากและหนังศีรษะส่วนหน้าข้างขวา

Material scraped from the floor of a vesicle was smeared on a slide and stained with Wright stain (รูป).

   1.การตรวจนี้มีชื่อเรียกว่า........................
Tzanck smear (scrapings from lesions stained with Giemsa or Wright stain)
2. ในรูป พบความผิดปกติอะไรบ้าง
Eosinophilic intranuclear inclusions และ multinucleated giant cell
3. การตรวจนี้ให้ผลบวกในโรคใดบ้าง
Herpes simplex, Varicella and herpes zoster, Pemphigus vulgaris, Cytomegalovirus

4. การวินิจฉัยคือ................................
คิดว่าเป็น herpes zoster เนื่องจากลักษณะเป็น unilateral vesicular eruption with a dermatomal distribution

   คุณหมอ Weeratian ตอบถูกต้องแล้วค่ะ ดิฉันก็เพิ่งทราบว่า Pemphigus vulgaris และ Cytomegalovirus ให้ Tzanck smear positive ด้วย

ผู้ป่วยรายนี้เป็น zoster ophthalmiticus ค่ะ

   แม้ว่า Tzanck test ได้ผลบวกพบได้ในการติดเชื้อ herpes simplex, herpes zoster และ varicella การกระจายตาม dermatome ของผื่นยืนยันว่าเป็น herpes zoster ที่เป็นกับ first branch ของ trigeminal nerve. ผู้ป่วยรายนี้ได้รับการรักษาด้วย oral acyclovir (800 mg five times per day for 10 days) ผื่นค่อยๆหายไปโดยไม่มี sequelae ใดๆ.
