A 56-YOM had swelling and pain in his anterior abdomen after a minor bicycle accident

ชายอายุ 56 ปีประสบอุบัติเหตุที่ไม่รุนแรงโดยโดยที่มือจับของรถจักรยานกระแทกที่ท้อง
ผู้ป่วยได้มาตรวจที่ห้องฉุกเฉินเนื่องจากหน้าท้องบวมขึ้นและเจ็บ เขามีประวัติเป็น atrial fibrillation และได้รับ oral anticoagulant therapy อยู่.

Physical examination revealed fullness and mild tenderness in his right periumbilical region, with a large pulsatile abdominal mass.

1. ในรูปคือ computed tomographic scan of the abdomen พบความผิดปกติอะไรบ้าง
2. จะ manage อย่างไรต่อไป

ขอตอบต่ะ
ใน CT scan ที่ลูกศรเล็กชี้เป็น hematoma ใน rectus muscle
ส่วนที่ลูกศรใหญ่ชี้ เป็น aorta ที่มีขนาดใหญ่ขึ้น (aneurysm) มี calcification ในผนังของ aorta ด้วยค่ะ

management: หยุด anticoagulant และปรึกษาศัลยกรรมค่ะ

CT abdomen revealed a right-sided hematoma of the rectus sheath, 8 cm by 6 cm (small arrow), with the incidental finding of an infrarenal abdominal aortic aneurysm, 7 cm in diameter (large arrow).

ผู้ป่วยได้รับการผ่าตัดเพื่อ repair abdominal aortic aneurysm และ evacuate rectus-sheath hematoma. ไม่มีภาวะแทรกซ้อนใดๆ ผู้ป่วยได้กลับบ้านวันที่ 5 หลังการผ่าตัด
