A 34-YOM presented with a 1-month history of anal and oral lesions




   ชายอายุ 34 ปี มารพ.เนื่องจากมีรอยโรคที่ทวารหนักและที่ในปากมานาน 1 เดือน
3 เดือนก่อนหน้านี้ เขามีแผลที่ foreskin ของ penis ซึ่งหายไปได้เอง

Physical examination revealed "snail-track" ulcerations in the mouth and on the lips and tongue as well as perianal lesions. Generalized lymphadenopathy was present, but the patient had no rash.

   -ผู้ป่วยมีรอยโรคที่ทวารหนักและที่ในปากมานาน 1 เดือน
-3 เดือนก่อนหน้านี้ เขามีแผลที่ foreskin ของ penis ซึ่งหายไปได้เอง น่าจะเป็น primary chancre= primary syphilis
-มี"snail-track" ulcerations in the mouth and on the lips and tongue as well as perianal lesions.
-Generalized lymphadenopathy

1. ผู้ป่วยน่าจะมี secondary syphilis perianal lesion นี้คือ condyloma lata ค่ะ
2. ถามประวัติการมีเพศสัมพันธ์ที่เสี่ยง homosexual หรือไม่
3. รายนี้ส่งตรวจ VDRL และ STD อื่นๆ ที่สำคัญคือตรวจ HIV antibody ด้วยค่ะ

   คำตอบของคุณหมอ lara ถูกต้องแล้วค่ะ

ผู้ป่วยเป็น bisexual man
HIV antibody positive CD4 cell count = 437 per cubic millimeter,

Titers of Treponema pallidum > 1:5120 on hemagglutination assay,
fluorescent treponemal antibody absorption test was positive.

แพทย์ได้ทำ lumbar puncture: cerebrospinal fluid revealed WBC 64 cells (predominantly mononuclear) per cubic millimeter and a nonreactive result on a Venereal Disease Research Laboratory test.

จะรักษาผู้ป่วยอย่างไร

   ให้การรักษาแบบ neurosyphilis โดยให้ PGS 5 ล้านหน่วย IV ทุก 6 ชม.x 14 วันค่ะ

   ผู้ป่วยได้รับการรักษาเหมือน neurosyphilis คือ ได้รับ intravenous penicillin G (24 million U per day) นาน 14 วัน รอยโรคที่ในปากและperianal lesions หายเป็นปกติ
