A 60-YOW presented with rapidly progressive swelling of left side of the face




   หญิงอายุ 60 ปี ซึ่งมีโรค metastatic neuroendocrine tumor มารพ.ด้วยเรื่องหน้าด้านซ้ายบวมมากขึ้นอย่างรวดเร็ว

PE: She was dehydrated and acutely ill, with a temperature of 36.1°C, a pulse of 106 per minute, and a blood pressure of 106/58 mm Hg. The left preauricular area of her face was markedly swollen, and a firm, tender mass was palpable.

The white-cell count was 54,400 per cubic millimeter, with 41 percent segmented granulocytes and 20 percent band forms.

   ขอตอบค่ะ
1. suppurative parotitis
2.
-ส่ง hemoculture และ pus culture 8jt
-ให้ broad spectrum antibiotic และ provide drainage
เชื้อที่พบบ่อยจะเป็น Staphylococcus aureus แต่เนื่องจากเป็นผป.immunocompromised host และผป.acutely ill+sepsis น่าจะ cover Gram-negative rod และ anaerobes ด้วยค่ะ

ดิฉันเลือก BL-BI คือ piperacillin+tazobactam ค่ะ ได้ผลเพาะเชื้อแล้วค่อยให้ยาตามผล sens ที่มีฤทธิ์แคบกว่า

   คุณหมอ lara ตอบถูกต้องแล้วค่ะ

กดที่ parotid (Stensen) duct ได้หนองออกมา.

CBC: white-cell count 54,400/cu mm, segmented granulocytes 41% และ band form 20%.

Computed tomographic scan of the head with the intravenous administration of contrast medium showed an enhancing mass within the left parotid gland, but no evidence of abscess formation.

ผู้ป่วยได้รับ broad-spectrum parenteral antibiotics

-การเพาะเชื้อจาก parotid-duct aspirate ขึ้น Escherichia coli และ Staphylococcus aureus.
-blood cultures ขึ้นเชื้อ S. aureus ซึ่งไวต่อยาต่างๆเหมือนกับเชื้อที่ได้ parotid duct.

3 วันต่อมา หน้าผู้ป่วยบวมมากขึ้น และมีหนองไหลออกมาจาก sinus tract ผู้ป่วยยังได้รับ intravenous antibiotic treatment ก้อนค่อยๆ ยุบลง โดยไม่ต้องผ่าตัด ผู้ป่วยได้กลับบ้าน 11 วันหลังจากเข้ารับการรักษาในรพ.

การติดตามต่อมา parotitis หายเป็นปกติ
