A 38-YOM with a family history of early CVA presented with a skin eruption

ชายอายุ 38 ปีซึ่งแข็งแรงดีมาก่อน มาพบแพทย์เนื่องจากมีผื่นที่แขนและ ลำตัวส่วนบน (รูป A และ B) ซึ่งเกิดขึ้นรวคเร็ว ไม่มีอาการผิดปกติอื่นๆ

เขามีประวัติคนในครอบครัวเป็น CVA ตั้งแต่อายุไม่มาก

0. การวินิจฉัยสำหรับรอยโรคที่ผิวหนังคือ......................................
2. จะส่ง investigate อะไรบ้าง

1.eruptive xanthoma ครับ
2.มีความสัมพันธ์กับกลุ่มโรคที่มี hypertriglyceridemia
ในผู้ป่วยรายนี้อายุน้อย มีปัญหาเรื่อง early CVA ในครอบครัว
ซึงเป็นลักษณะของ premature atherosclerosis ดังนั้นจึงต้องหา familial hyperlipidemia โดยเฉพาะ type I , IV , V ซึ่งจะมาด้วย eruptive xanthoma
investigate เบื้องต้น : overnight standing tube test , lipid profile
esp TG ครับ

รูป A และ B: eruption of pink papules with creamy-colored centers on his arms and upper torso.

ผลของ laboratory tests ทำเมือผู้ป่วยอดอาหาร:
-total cholesterol level of 1268 mg per deciliter,
-total triglyceride level of 8869 mg per deciliter
-glucose level of 241 mg per deciliter, และ
-glycosylated hemoglobin value of 13.4 percent.

จะรักษาผุ้ป่วยอย่างไรดีคะ

ขอตอบค่ะ
1. รักษา hypertriglyceridemia ก่อนค่ะ ด้วย gemfibrosil ส่วนการรักษา hypercholesterolemia รอไว้ก่อนค่ะ
2. รักษา DM เพราะ HBA1C สุงมากและผป.อายุน้อย ขอให้เป็น insulin ก่อนค่ะ

คุณหมอ lara ตอบถูกต้องแล้วค่ะ

ได้เริ่มให้การรักษาเพื่อลด triglyceride ด้วย gemfibrozil (enhance lipoprotein lipase activity), และรักษาเบาหวานด้วย insulin therapy

24 ชั่วโมงต่อมา total triglyceride level ลดลงเป็น 6466 mg/dL .

จากการติดตามที่ 3 เดือนและ 5 เดือนต่อมา eruptive xanthomas มีจำนวนลดลงและขนาดเล็กลง

หลังจากรักษาด้วย gemfibrozil, glyburide และ glucophage นาน 9 เดือน total cholesterol level 218 mg/dL, total triglyceride level 302 mg/dL, และ glycosylated hemoglobin value 8.3 percent; eruptive xanthomas หายไปหมด.

เมื่อทบทวนเวชระเบียนของผู้ป่วย พบว่าเขาเคยมี preexisting, untreated dyslipidemia (total cholesterol level, 216 mg/dL; total triglyceride level, 422 mg/dL), ซึ่งเข้าได้กับ Frederickson type IV familial hypertriglyceridemia. อย่างไรก็ตาม ในผู้ป่วยรายนี้ ควรจัดเป็น type V phenotype, mixed hypertriglyceridemia มากกว่าเนื่องจากมี new-onset diabetes mellitus ร่วมด้วย.
