A 45-YOM with AIDS had night sweats, fatigue, cachexia, and productive cough

PE: temperature, 40°C, heart rate, 136 per minute, and blood pressure, 90/50 mm Hg
CBC: leukocyte count of 2.5x103 per cubic millimeter
Chest x-ray film revealed an infiltrate in the right lower lobe.

1. พบ yeast cells จำนวนมากอยู่ใน monocyte ซึ่งอาจเป็น histoplasma, Penicillium marneffei หรือ Leishmania spp. แต่เพราะไม่พบ clear septum (Penicillium marneffei ) และ kinetoplast (Leishmania spp.) จึงน่าจะเป็น Histoplasma capsulatum ค่ะ
2. dissseminated histoplasmosis ค่ะ

peripheral-blood smear (Wright stain, x1000) showed a monocyte with a grossly distorted nucleus and numerous intracellular yeast-like organisms 2 to 4 µm in diameter with eccentric chromatin. The organisms were surrounded by an artifactual pseudocapsule caused by cytoplasmic shrinkage. These features are diagnostic of Histoplasma capsulatum. Of 200 neutrophils or monocytes examined, 26 (13 percent) contained these organisms, and the intracellular burden ranged from 1 to 20 organisms.

เพาะเชื้อจากเลือดตั้งแต่แรกรับขึ้น H. capsulatum

ผู้ป่วยได้รับการรักษาด้วย amphotericin B, แต่สภาพของผู้ป่วยทรุดลงและเสียชีวิตจาก septic shock 2 สัปดาห์ต่อมา.

คนเราได้รับ H. capsulatum จากการหายใจเอา conidia หรือ mycelial fragments เข้าไป. Reactivation ของ latent infection ก็เป็นไปได้
