An 86-YOW with recurrent UTIs had vague abdominal symptoms

หญิงอายุ 86 ปีซึ่งมีประวัติ recurrent urinary tract infections เป็นเวลาหลายปี มารพ.ด้วยอาการปวดทั่วๆท้อง
Physical examination: right-flank tenderness.

ผู้ป่วยมีโรคติดเชื้อทางเดินปัสสาวะที่เกิดจาก Escherichia coli. ได้ทำ contrast-enhanced computed tomographic (CT) scan (เนื่องจากการตรวจพบโดย ultrasonography และ radiography ไม่ได้คำตอบ) พบ typical finding ของ xanthogranulomatous pyelonephritis ในไตข้างขวา (รูป A).

Xanthogranulomatous pyelonephritis เป็นการติดเชื้อแบคทีเรียที่เรื้อรัง bacterial ของไต มีลักษณะที่สำคัญคือ มีการทำลายเนื้อไตและมี granulomas, abscesses, และมี cellular infiltrates ของ lipid-laden macrophages (foam cells). มีก้อนหลายก้อน (multiple masses, M) อยู่แทนที่เนื้อไตและออกไปสู่ perinephric space (PN). ผนัง (W)ของก้อนเหล่านี้ถูก enhanced, และบางก้อนก็มีนิ่ว (calculi, C).

ผู้ป่วยได้กลับบ้านพร้อมยา ofloxacin

6 เดือนต่อมา ผู้ป่วยได้มาตรวจเนื่องจากไปชนขอบโต๊ะและคลำพบก้อนขนาดใหญ่ที่ right flank.

CT scan (รูป B) พบความผิดปกติอะไรบ้าง

Findings : มี right renal capsule perforate และมีส่วนของ content leakage ออกมายังบริเวณของ retroperitoneal space
Adequate treatment : Surgical drainage แล้วนำ pus มา culture คิดว่าน่าจะ nephrectomy ด้วยในผู้ป่วยรายนี้เพราะมีภาวะที่เป็น complicated condition แล้วครับ

การเพาะเชื้อของหนองที่ระบายจากก้อน ขึ้น Streptococcus constellatus
ผู้ป่วยได้รับการผ่าตัดตัดไตข้างขวา
ผู้ป่วยสบายดีหลังจากนั้นจากการติดตามที่ 5 ปีหลังการผ่าตัด

ขอถามต่อค่ะ Streptococcus constellatus เป็นเชื้อในกลุ่มใด

Streptococcus constellatus, S. intermedius,และ S. anginosus
(ถูกขัดอยู่ใน the S. milleri group) เป้นส่วนหนึ่งของ normal flora ของช่องปาก, gastrointestinal tract และ genitourinary tract และมักจะสัมพันธ์กับpurulent infection ค่ะ
