An 88-YOW with a painless, nonpulsating suprapubic mass

หญิงอายุ 88 ปีซึ่งถูกพบว่านอนอยู่ที่พื้นใกล้กับรถเข็นใน nursing home ได้ถูกนำส่งรพ.เนื่องจากมี upper GI bleeding.

On physical examination, a painless, nonpulsating mass was palpable in the suprapubic region.

แพทย์ได้ส่ง radiograph of the pelvis เนื่องจากต้องการดูว่ามี fracture ของ pelvis หรือ hip หรือไม่

1. พบความผิดปกติอะไรใน radiograph of the pelvis นี้
2. บอกการวินิจฉัยแยกโรค

ขอลองตอบค่ะ

radiograph ของ pelvis ที่เด่นมากคือมี calcified round mass ที่ suprapubic area โดยอยู่ค่อนไปทางขวา

นึกถึงความผิดปกติของอวัยวะที่อยู่บริเวณนี้ (ซึ่งอาจเป็นทั้ง benign และ malignant conditions)
-gynecologic (uterus, adnexa, ovary )
-urinary bladder (tumor, stone)
-bowels
-อยู่ในช่องท้องนอก organ เหล่านี้

ไม่แน่ใจว่าการวินิจฉัยเป็นอะไรค่ะ

Radiograph of the pelvis showed a large, calcified mass.

การวินิจฉัยแยกโรคของ calcified pelvic masses มีอยู่มากทั้ง benign และ malignant entities เช่น calcifications ของ aneurysms, urinary stones, calcified soft tissues หลังจากมีเลือดออกหรืออักเสบ, calcified foreign bodies, lithopedia, osseous metaplasia ของ myometrium, และ calcified tumors.

อย่างไรก็ตาม, การมี dense, well-defined calcification โดยมีขนาดและตำแหน่งดังในผู้ป่วยนี้เป็น pathognomonic ของ calcified leiomyoma ของมดลูก ดังนั้น จึงไม่จำเป็นต้องตรวจเพิ่มเติมหรือให้การรักษาใดๆ
