A 45-YOM had fever, myalgia, and headache

ชายอายุ 45 ปี ได้เข้ารับการรักษาในรพ.เพราะมีไข้สูง (T 40°C) ปวดกล้ามเนื้อทั้งตัวและปวดหัว ผู้ป่วยไม่ได้มีอุจจาระเหลว

1. จากการตรวจร่างกาย ดังในรูป A พบความผิดปกติอะไรบ้าง

การตรวจร่างกายอื่นๆ อยู่ในเกณฑ์ปกติ

CBC: WBC 17,500/cumm และ 14 % eosinophils (2450/cumm)

2. การวินิจฉัยคือ ..........................................................

ขอตอบค่ะ

pertinent data คือ
1. ชายอายุ 45 ปี
2. fever, generalized myalgia, headache
3. eosinophilia (2450/cumm)
4. ความผิดปกติทางตา คือ chemosis อย่างมาก มี mild periorbital edema

การที่มี combination ของ fever, generalized myalgia, periorbital edema +eosinophilia ต้องนึกถึง trichinosis.

ต้องถามประวัติการกินเนื้อดิบหรือสุกๆ ดิบๆ มีผู้ที่กินอาหารด้วยกันมีอาการเหมือนผู้ป่วยหรือไม่

confirm การวินิจฉัยด้วยการทำ muscle biopsy และ serologic test

คุณหมอ lara เก่งมากค่ะ ตอบถูกต้องแล้วค่ะ

เขามี bilateral proptosis with chemosis และ eyelid edema เขาไม่มี trismus หรือ subungual splinter hemorrhages.

Serum creatine kinase level = 651 U/L (N < 170 U/L).

Initial serologic test for trichinosis โดยใช้ enzyme-linked immunosorbent assay ได้ผลลบ (ได้ค่า 0.09; ค่า <0.3 แสดงว่าเป็น negative result for trichinosis, และค่าที่ >0.5 เป็น positive result).
CT scan of the orbits (รูป B): bilateral proptosis (the posterior part of the eyes, indicated on one side by an arrowhead, is normally at the level indicated by the arrow below it) and moderate ocular-muscle hypertrophy (arrows).

แพทย์นึกถึง trichinosis เพราะมีการระบาดของ trichinosis ในเมือง Toulouse ฝรั่งเศสในขณะนั้น. ซักประวัติจากผู้ป่วยได้ว่าเขาได้กินเนื้อม้าดิบ 10 วันก่อนมีอาการ.

จะให้การรักษา trichinosis อย่างไร

ยาที่ใช้รักษา trichinosis คือ albendazole (400 mg twice a day for 8-14 days, ยานี้ไปยับยั้ง microtubule formation และมี “larvicidal effects”) ร่วมกับprednisolone (20-60 mg per day 2-3 วันแรก) เพื่อลด inflammation ค่ะ

คุณหมอ lara ตอบได้ถูกต้องแล้วค่ะ

ผู้ป่วยได้รับการรักษาด้วย prednisolone (1 mg/kg/d) และ albendazole (10 mg/kg) นาน 10 วัน อาการแสดงทางตาดีขึ้นอย่างรวดเร็ว

2 เดือนต่อมา serologic test สำหรับ trichinosis กลายเป็นบวก (ค่า 2.25) ซึ่งขณะนั้น ผู้ป่วยไม่มีอาการผิดปกติใดๆ
