A 45-YOW with persistent fatigue




   หญิงอายุ 45 ปีซึ่งมีผิวหนังด่างขาวดังในรูป A มานาน 17 ปี ได้มาตรวจที่รพ.เนื่องจากเพลียมานานเป็นเดือน
ตรวจร่างกาย พบว่าผู้ป่วยซีด นอกจากผิวหนังด่างขาวแล้ว อื่นๆ ปกติ

CBC: white-cell count 3200/cumm; hemoglobin level, 8.1 g/dL; platelet count 147,000/cumm) with mean corpuscular volume 125 µm3.

   ข้อแรกตอบว่าเป็น Vitiligo ค่ะ
ข้อที่2 เป็น hypersegmented neutrophil ผู้ป่วยน่าจะเป็น megaloblastic anemia ค่ะ ก็เลยทำให้เกิด persistent fatigue
พอดีไปค้นมาเจอว่า ถ้าเป็น vitiligo + megaloblastic anemia
น่าจะเป็น pernicious anemia
เพราะฉะนั้นข้อ3 ตอบว่า pernicious anemia ค่ะ

   คุณหมอ dot ตอบถูกต้องแล้วค่ะ

megaloblastic anemia อาจเกิดจากการขาด folate หรือ B12
serum vitamin B12 level =84 pmold/L liter (normal range, 220 to 660).
serum folate level was normal.

   Bone marrow aspirate: erythroid hyperplasia with left-shifted megaloblastic erythropoiesis and giant metamyelocytes (รูป C, arrow; hematoxylin and eosin, x100).

Gastric biopsy revealed severe alterations of the mucosa with glandular atrophy and intestinal metaplasia.

การวินิจฉัย chronic atrophic gastritis type A (autoimmune) มีข้อมูลสนับสนุนคือ พบว่ามี markedly elevated serum levels of antibodies to gastric parietal cells (1:260; normal value, <1:10).

ผู้ป่วยได้รับการรักษาด้วย intramuscular vitamin B12 และต่อมา ความผิดปกติทางเลือดกลับเป็นปกติ

   มีหลักฐานมากขึ้นที่สนับสนุนว่า vitiligo เป็นความผิดปกติทาง autoimmune และอาจสัมพันธ์กับโรค autoimmune อื่นๆ เช่น pernicious anemia, rheumatoid arthritis, type 1 diabetes, alopecia areata และ โรคต่างๆ ของต่อมไทรอยด์ หรือ vitiligo อาจเป็นอาการแสดงอย่างหนึ่งของ polyglandular autoimmune syndrome.
