A 39 YOW had a sinus at web between middle and ring fingers for 5 mos

หญิงอายุ 39 ปีซึ่งมีสุขภาพดี ทำงานเลี้ยงสัตว์ มาหาแพทย์เนื่องจากมีน้ำซึมๆ ออกมาจากรูเล็กๆ ที่ง่ามระหว่างนิ้วกลางและนิ้วนางของมือขวามานาน 5 เดือนโดยไม่ตอบสนองต่อการรักษาด้วยยาปฏิชีวนะ .

Examination revealed a small sinus tract with visible hair (รูป A, ลูกศร) and an area of chronic inflammation (หัวลูกศร).

ขอตอบว่า การวินิจฉัยคือ pilonidal sinus ค่ะ มีขนแทงเข้าไปที่ง่ามนิ้วมือทำให้เกิดเป็น pilonidal cyst
การรักษา ต้อง excise เอา cyst ออกค่ะ

การวินิฉัยคือ interdigital pilonidal sinus

ผู้ป่วยได้รับการผ่าตัดเอา area of chronic inflammatory tissue with a sinus tract and multiple visible hairs ออก (รูป B).

Pathological examination of the specimen (รูป C; hematoxylin and eosin, x40) revealed a sinus tract with abundant keratin (upper left portion of panel), epidermal thickening, dermal fibrosis with chronic inflammation (lower right portion of panel), and multiple embedded hairs (arrows).

ผู้ป่วยหายดีและไม่มีน้ำซึมออกมาอีกเลยหลัง excision.

คำว่า "pilonidal" มาจากภาษา Latin ว่า pilus (แปลว่า ขน) และ nidus (แปลว่า nest).

Pilonidal cysts สามารถเป็นที่ใดของร่างกายก็ได้ และ ในช่างตัดผมและคนเลี้ยงสัตว์ อาจพบ pilonidal cysts ได้ที่ง่ามนิ้วมือ เมื่อขนเส้นหนาๆ ฝังเข้าไปที่ interdigital fold ก็อาจทำให้เกิดการอักเสบเรื้อรัง กลายเป็น sinus tract และมี chronic purulent drainage การผ่าตัดเอาขนและ inflammatory tissue ออก ทำให้ pilonidal cysts หายไป
