A 26-YOM with whitish oral lesions and odynophagia




   ชายอายุ 26 ปีมาตรวจที่รพ.เนื่องจากมีรอยโรคสีขาวๆที่ในปาก (รูป A และ B) และเจ็บเวลากลืนอาหาร เขาสังเกตว่ามีแผลที่ penis เมื่อ 7 เดือนก่อน และแผลหายไปเอง
เขาไม่มีไข้และมีสุขภาพดี

Physical examination revealed pseudomembranous lesions and erosions of the tongue, the hard and soft palate, and tonsils, as well as generalized lymph-node enlargement.

   ขอลองตอบครับ
--->ข้อ 1.mucous white patch lesion เหมือน syphilis มากที่สุด เพราะ 1)มีประวิติ STD (แผลที่ penis)
2)มีลักษณะ discrete, small, white necrotic lesion ที่ tongue, palate หรือ lips (ในขณะที่ candidiasis
มักจะ diffuse มากกว่า) ใน secondary syphilis หากมี positive serologic findings ก็จะแยกจาก candidiasis ได้
--->ข้อ 2 ส่ง investigation คือ darkfield examination or immunofluorescent staining of mucocutaneous lesions
หากเป็น secondary syphilis จริง ก็ treat โดย Benzathine penicillin G † 2.4 million units IM in a single dose หรือหากแพ้ penicillin ก็ให้ Doxycycline[*] 100 mg PO bid for 15 days, or tetracycline hydrochloride,[*] 500 mg PO qid for 15 days; or Azithromycin 2.0 g single dose ‡ or desensitization to penicillin in pregnant women

   คุณหมอ Weeratian ตอบถูกต้องแล้วค่ะ เก่งมากค่ะ

ตรวจร่างกาย ไม่พบ condylomata lata, macular skin lesions หรือ alopecia.

Titers of Treponema pallidum were 1:81,920 on hemagglutination assay, and a fluorescent treponemal antibody absorption test was positive.
A serologic test for human immunodeficiency virus infection was negative.

ผู้ป่วยได้รับการรักษาด้วย intramuscular penicillin G benzathine (2.4 million U) ครั้งเดียว oral lesion หายไปหมดภายในเวลา 2 สัปดาห์.
