ชาย 42 ปี มีไข้ น้ำหนักลด มา 2 เดือน ร่วมกับปวดบวมที่แขนและต้นขาขวา




   ชายอายุ 42 ปี มีไข้ต่ำๆ มานาน 2 เดือนร่วมกับมีน้ำหนักลด 14 กก. ปวดและบวมที่แขนและต้นขาขวาโดยไม่มีประวัติ trauma.
4 เดือนก่อน เขามีฝีที่รากฟัน.
รูป A คือ Gadolinium-enhanced magnetic resonance imaging ของ right thigh

3. เชื้อก่อโรคน่าจะเป็นเชื้ออะไรมากที่สุด

   ขอลองตอบนะคะ
Pertinent data
1. ไข้ต่ำๆ 2 เดือน น้ำหนักลด 14 กก.
2. ปวดและบวมที่แขนและต้นขาขวา
3. มีฝีที่รากฟันเมื่อ 4 เดือนก่อน

รายนี้ต้องเช็คว่าเขาเป็น normal host หรือไม่ ควรทำ anti HIv antibody ด้วย

ถ้าเป็น normal host มี chronic infection involve แขนและต้นขาขวา ลักษณะการติดเชื้อน่าจะเป็น hematogenous เขาอาจมี IE แต่ในโจทย์ไม่ได้ให้ข้อมูลมา
ส่วนเชื้อก่อโรคที่น่าจะเป็นคือ
1.เชื้อที่เกี่ยวกับฟัน ก็คิดว่าน่าจะเกิดจาก viridans streptococci หรือ oral anaerobes รวมถึง Actinomyces ด้วย
2. เชื้ออื่นๆที่ทำให้เกิดการติดเชื้อหลายแห่ง เช่น TB, systemic fungal infection เช่น penicillosis, histoplasmosis เป็นต้น

สำหรับ Gadolinium-enhanced MRI มี area ดำๆ เหมือนเป็น abscess น่าจะอยู่ในกระดูกนะคะ
Radiography ของ right arm: มี periosteal elevation ที่ลูกศรสั้น
Bone scan: increased uptake ที่ right femur และ right humerus

เขาน่าจะมี Osteomyelitis ที่ right femur และ right humerus
ส่วนเชื้อสาเหตุ ก็ต้องเจาะ right femur มาย้อม Gram, Wright, modified acid fast และ acid fast stain และส่ง hemoculture ด้วยค่ะ

Gadolinium-enhanced MRI: a large area of decreased signal intensity (long arrow) with several enhancing septa and periosteal elevation with loss of bone integrity (short arrow).
Radiography ของ right arm: a periosteal reaction (short arrow) and intramedullary gas (long arrow).
Bone scan (Panel C) showed increased uptake in the right femur, proximal right humerus, and left scapula.

Drain หนองจากต้นขาขวา ได้หนอง 4000 มล. เพาะเชื้อของหนองขึ้น fusobacterium.

ผู้ป่วยตอบสนองดีต่อการรักษาทางศัลยกรรม (surgical débridement) และ amoxicillin-clavulanate นาน 8 สัปดาห์.

สาเหตุของ bacteremia น่าจะเป็นจากการติดเชื้อที่ฟัน.

Hematogenous anaerobic osteomyelitis อาจสัมพันธ์กับ trauma และ fracture, human bites, dental infections, chronic sinusitis, diabetes, diverticulitis และแผลกดทับ
