เด็กชาย 6 ปี มีไข้และ flaccid bullae มา 2 วัน




   เด็กชายอายุ 6 ปี เข้ารับการรักษาในรพ.เนื่องจากมีไข้และผื่น.
ปกติเขาแข็งแรงดี.
2 วันก่อน เขาเริ่มคันทั้งตัว และเริ่มมีตุ่มน้ำเล็กๆเกิดขึ้นที่จมูกและหน้า. ในช่วง 2 วันนี้ รอยโรคลุกลามมากขึ้นจนเป็นทั่วตัว ผู้ป่วยได้รับ cephalexin เมื่อ 1 วันก่อน

PE: temperature was 38.3°C on admission. He appeared extremely uncomfortable but not acutely ill. He had numerous flaccid bullae on his face, neck, axilla, perianal region, upper back, and thighs. Nikolsky sign was present (ready separation of the outer layer of epidermis from the basal layer)

   ขอลองตอบ ไม่ทราบว่าถูกรึป่าว เพราะทราบเพียงโรคเดียว
"Staphylococcal Scalded Skin Syndrome" มีการหลุดลอกของ skin
ตอนนี้เรียนอยู่ที่ NICU ทราบว่าในเด็กแรกเกิดเรียกโรคนี้ว่า Ritter’s disease
ซึ่งเกิดจาก S.aureus
-->หากข้อ 1&2 ตอบถูก การรักษาคือ
1.Intravenous penicillinase-resistant penicillin (e.g., nafcillin, 100 mg/kg/day for newborns, 100 to 200 mg/kg/day for older children)
2.สำหรับ Topical treatment ให้เป็น cool saline compresses
ส่วน Systemic corticosteroids เพียงอย่างเดียวนั้นไม่ควรให้เพื่อรักษา SSSS

   คุณหมอ Weeratian ตอบได้ถูกต้องแล้วนะคะ ขอบคุณที่ให้ข้อมูลการรักษามาด้วยค่ะ

skin biopsy ของรอยโรค พบว่า level of cleavage อยู่ที่ granular layer.

ผู้ป่วยได้รับการรักษาด้วย IV cloxacillin และอาการต่างๆดีขึ้นใน 3 วันต่อมา

เขาได้กลับบ้าน ในวันที่ 7 ของการเข้าอยู่ในรพ. โดยผื่นหายไปหมด.
