ชาย 60 ปี อ่อนแรง น้ำหนักลดลงมากและมีไข้ต่ำๆ มา 2 เดือน




   ชายอายุ 60 ปี อาชีพเลี้ยงไก่ไ ด้เข้ารับการรักษาในรพ. เนื่องจากอ่อนแรง น้ำหนักลดลงมากและมีไข้ต่ำๆมานาน 2 เดือน.

ตรวจร่างกาย: He had cachexia with diffuse muscle weakness, exaggerated myotatic reflexes, bilateral ankle clonus, and Babinski signs. He did not have hepatosplenomegaly, lymphadenopathy, or mucocutaneous lesions.

ผู้ป่วยไม่ได้กินยากดภูมิต้านทานใดๆ และการตรวจ HIV antibody ได้ผลลบ แต่ CD4 cell count 238/cumm.

Chest radiograms ปกติ.

ในรูปคือ T1-weighted magnetic resonance imaging ของสมอง พบความผิดปกติอะไรบ้าง

   ใน MRI brain พบ ring-enhancing lesions จำนวนมาก ที่ cerebrum, cerebellum และ brainstem.

ผู้ป่วย non-HIV แต่ CD4 cell count 238/cumm. น่าจะมีการติดเชื้อต่างๆ หรือ malignancy ได้เหมือนผป.ติดเชื้อ HIV ที่นึกถึงคือ

OI: toxoplasmosis, TB,
Infection อื่นๆ เช่น cysticercosis, bacterial infection (เช่น brain abscess), fungal infections

T1-weighted magnetic resonance imaging ของสมอง: multiple ring-enhancing lesions of the cerebrum, cerebellum, and brain stem (larger lesions are indicated by arrowheads in Panel A).

ตรวจ cerebrospinal fluid พบ 7 white cells/cumm (44 % neutrophils, 44 % lymphocytes, 7 % monocytes, 3 % macrophages, and 2 % eosinophils), 2080 red cells/cumm, a glucose level of 74 mg/dL and a protein level of 54 mg/dL.

Gram staining และ การเพาะเชื้อ cerebrospinal fluid ได้ผลลบ และ immunoassays ได้ผลบวกต่อ histoplasma antigen และได้ผลลบต่อ cryptococcus antigen.

Stereotactic brain biopsy: numerous foci of granulomatous lymphohistiocytic inflammation surrounding necrotic tissue.

   histology ของ brain biopsy (Gomori methenamine silver stain) พบ yeast cell จำนวนมาก และมี budding ขอตอบว่าเป็น Histoplasma capsulatum ค่ะ

histology ของ brain biopsy (Gomori methenamine silver stain)
shows a representative lesion with numerous organisms, some of which were budding (arrow), whose appearance was consistent with that of Histoplasma capsulatum.

ผู้ป่วยได้รับการรักษาด้วย intravenous amphotericin B (total dose, 2 g), และการรักษาได้เปลี่ยนเป็นกิน fluconazole (400 mg/วัน) นาน 6 เดือน. อาการของผู้ป่วยดีขึ้นเรื่อยๆ ที่ 6 เดือนหลังวินิจฉัย เขาสามารถเดินได้โดยใช้ไม้เท้า

   จากตำราโรคติดเชื้อ บรรยายลักษณะของ Histoplasma capsulatum ว่ามีลักษณะเป็นยีสต์ตัวรี มีแตกหน่อ (budding) บางครั้งเหมือนพวงองุ่น
