ชาย 57 ปี มี contracture ของนิ้วทั้ง 2 มือ




   ชายอายุ 57 ปีซึ่งเป็นเบาหวานและดื่มสุรามานานถูกส่งตัวมา hand surgery unit เนื่องจากมี contracture ของนิ้วทั้ง 2 มือ. พ่อและน้องของผู้ป่วยมีลักษณะ contracture แบบนี้เหมือนกัน.

Physical examination (รูป) revealed flexion contractures involving the bilateral third digits and the right fifth digit. The patient had a severe contracture of 100 degrees at the proximal interphalangeal joint of the right small finger and thickened palmar fascia with multiple cords ending in firm nodules.

   1. เห็นด้วยครับว่าเป็น Dupuytren contraction
2. risk factor:
1.male:female ratio of 10:1
2.40-60 year
3.Family history (autosomal dominant pattern of inheritance with incomplete penetrance)
4.Alcohol
5.Diabetes
6.Smoking
(trauma or epilepsy, are not well defined.)

Added:
Diabetes was a significant risk factor (OR=1.75) and there was an increased risk for medicinally treated diabetes (metformin – R=3.56; sulphonylureas – OR=1.75) and particularly insulin controlled (OR=4.39) rather than diet-controlled diabetes. Epilepsy (OR=1.12)

Dupuytren อ่านว่า Du-pwe-trahn เป็นชื่อของ Baron Guillaume Dupuytren ศัลยแพทย์ชาวฝรั่งเศส (1777-1835)

การวินิจฉัย: Dupuytren contracture, ซึ่งเป็น fibroproliferative disorder ของ palmar fascia.
ต่อมาผู้ป่วยได้รับการผ่าตัด bilateral partial fasciectomy.

Risk factors ที่พบบ่อยของ Dupuytren contracture คือ ประวัติครอบครัวที่เป็นโรคนี้, เบาหวาน, การติดสุรา และการใช้เครื่องมือที่สั่นสะเทือน

ในผู้ป่วยรายนี้ มือหายดี และไม่กลับเป็นซ้ำอีกจากการติดตามระยะยาว
