ชาย 69 ปี ปวดท้อง 36 ชั่วโมงหลังการตรวจ colonoscopy และ polypectomy.

ชายอายุ 69 ปีมาตรวจที่ห้องฉุกเฉินเนื่องจากปวดที่ท้องส่วนล่างข้างซ้ายและที่ขาหนีบมานาน 3 วัน. ผู้ป่วยเริ่มปวดท้อง 36 ชั่วโมงหลังจากได้รับการตรวจ colonoscopy ซึ่งพบว่าเขามี extensive diverticular disease และแพทย์ได้ทำ polypectomy.

Physical examination: HR 110/min, blood pressure 95/70 mm Hg and temperature 38.2°C, erythema and swelling of the left inguinoscrotal region with associated tenderness on palpation. The remainder of his abdomen was generally soft and nontender on palpation.

Laboratory testing อยู่ในเกณฑ์ปกติ ยกเว้น
creatine kinase level 387 U/L (normal range, 0-170),
international normalized ratio = 1.4, และ
derived fibrinogen level = 6.4 g/L (normal range, 1.5 - 4.5).

ชาย 69 ปี
Problems
1. ปวดที่ท้องส่วนล่างข้างซ้ายและที่ขาหนีบมานาน 3 วัน (PE: erythema swelling and tenderness of the left inguinoscrotal) อาการปวดท้องสัมพันธ์กับ colonoscopy+polypectomy (36 ชั่วโมงหลัง procedure)
2. extensive diverticular disease
3. Sepsis with impending shock (HR 110/min, blood pressure 95/70 mm Hg and temperature 38.2°C)
4. creatine kinase level 387 U/L (normal range, 0-170) เขาอาจมี necrotizing fasciitis
5. DIC? [international normalized ratio = 1.4, และ
derived fibrinogen level = 6.4 g/L (normal range, 1.5 - 4.5)].

รูป B ที่ลูกศรชี้ มี gas อยู่ใน retroperitonuem

Dx: perforation of colon by polypectomy --> infection involve retroperitoneum and skin+soft tissue--> scrotum --> sepsis-->septic shock with DIC

Management:
1 Fluid resuscitation
2. Take blood culture, ถ้ามี หนอง ก็เจาะหนอง ทำ Gram stain และส่ง culture
3 ให้ broad spectrum antibiotic cover GNR และ anaerobes
4. ปรึกษาศัลยกรรมด่วน

An axial view shows diverticular disease affecting the distal colon and widespread gas in the retroperitoneal tissues, spreading along the left renal fascia (รูป B, arrows) and extending into the left hemiscrotum, causing gangrene of the scrotum and perineum (รูป C).

CT images ของ abdomen และ pelvis ที่ทำต่อมา: marked diverticular disease affecting the distal colon and widespread gas in the retroperitoneal tissues, spreading along the left renal fascia (รูป B) and along the left psoas muscle down the left paracolic gutter, extending to the anterior abdominal wall and into the left hemiscrotum.

การวินิจฉัย: Fournier gangrene secondary to retroperitoneal perforation of the colon during colonoscopy

ผู้ป่วยได้รับการผ่าตัด immediate exploratory laparotomy พบว่ามี necrosis ของ left retroperitoneal tissues ใกล้และอยู่ต่ำกว่า sigmoid colon ไม่พบรูทะลุของ colon ได้ทำ Hartmann procedure พบว่า erythema ของ inguinoscrotal region ซึ่งมีอยู่ตอนแรกรับได้กลายเป็น gangrene ของ scrotum และ perineum ชัดเจน (รูป C), ต้องทำ radical debridement ซ้ำ

ไม่ทราบแน่ชัดว่าการทะลุของลำไส้เกิดขึ้นที่ diverticulum หรือที่ตำแหน่งของ polypectomy.
