ชาย76 ปี ตาขวามองไม่เห็นทันที




   ชายอายุ 76 ปี ซึ่งมี hypertension มารพ.ด้วยอาการตาขวามองไม่เห็นทันทีโดยไม่มีอาการปวด
Visual acuity: light perception only in the right eye, with a positive relative afferent pupillary defect.

1. ในรูป A คือ ภาพของ retina จากการตรวจ ophthalmoscopy พบความผิดปกติอะไรบ้าง
2. การวินิจฉัยโรคคือ...................................................................
3. จะ manage อย่างไร

   ผู้ป่วยมี sudden onset of vision loss ของตาขวา
1. ความผิดปกติที่เห็นน่าจะเป็นคือ cherry red spot ค่ะ
2. การวินิจฉัย คือ central retinal artery occlusion

   คุณหมอ Lara ตอบได้ถูกต้องแล้วค่ะ

Ophthalmoscopic examination: diffuse retinal whitening, constriction of the arteriole and venule with segmentation (รูป A, หัวลูกศร), and a cherry-red spot in the macula (รูป A, ลูกศร) — อาการแสดงทั้งหมดเข้าได้กับการวินิจฉัย central retinal artery occlusion.

A horizontally oriented optical coherence tomograph of the macula (รูป B, หัวลูกศร) demonstrated increased thickness and reflectivity of the inner bands, indicating ischemic damage to the nerve fiber and adjacent layers (รูป B, ลูกศร).

   การรักษา central retinal artery occlusion ยังไม่ทราบกัน. เนื่องจากเป็นภาวะที่ทำให้สูญเสียการมองเห็น จึงได้ทำ ocular massage และ paracentesis ของ anterior chamber แต่ไม่ได้ทำให้การมองเห็นของผู้ป่วยดีขึ้น.

การตรวจหาสาเหตุที่ทำให้เกิด central retinal artery occlusion เช่น erythrocyte sedimentation rate, C-reactive protein level, และ echocardiogram อยู่ในเกณฑ์ปกติ. Ultrasonography ของ carotid arteries แสดงให้เห็น stenosis น้อยกว่า 50% ข้างขวา.

Central retinal artery occlusion สัมพันธ์กับภาวะต่างๆทางอายุรกรรม ซึ่งรวมถึง embolic events และ rheumatologic conditions เช่น temporal arteritis.

เนื่องจากไม่พบสาเหตุใดๆ ในผู้ป่วยรายนี้ ผู้ป่วยจึงได้รับเพียง antiplatelet therapy. พบว่ามี optic atrophy ภายใน 12 สัปดาห์ และผู้ป่วยมองเห็นแต่แสง (light perception) ด้วยตาข้างขวา
