เด็กหญิง4.5 ปี ตัวเล็กและมีก้อนที่โคนลิ้น




   
เด็กหญิงอายุ 4.5 ปี แม่พามาตรวจเพื่อประเมินการเจริญเติบโต.
การตรวจร่างกายทั่วไปอยู่ในเกณฑ์ปกติยกเว้นพบก้อนที่โคนลิ้นของผู้ป่วย (ลูกศร). ส่วนสูงและน้ำหนักของเด็กอยู่ที่ 25th percentile สำหรับอายุและเพศ

   ตอบใหม่ค่ะ คิดถึง lingual thyroid ค่ะ

Ectopic lingual thyroid is an uncommon embryological aberration characterised by the presence of thyroid tissue located in a site other than the normal, pretracheal region. The tongue is the most frequent ectopic location of the thyroid gland; the clinical incidence of lingual thyroid varies between 1:3000 and 1:10,000. We present a new case of lingual thyroid in a 10-year-old child who presented dysphagia. The radiological findings and the biopsy confirmed the diagnosis. As the mass was well tolerated, surgery was not indicated. At this time, there are no signs of complications or malignant transformation.

ได้ส่ง thyroid function test ได้ผลดังนี้
-serum thyrotropin level 55 mU/L (normal range, 0.7- 6.4)
-thyroxine level 3.5 µg/dL (normal range, 6.0 - 14.2)

[99mTc]sodium pertechnetate scan: increased 99mTc uptake at the base of the tongue, which was consistent with the presence of a lingual thyroid; there was no uptake over the neck.

ผู้ป่วยได้รับการรักษาด้วย levothyroxine สำหรับ hypothyroidism.
หลังจากนั้น เมื่อได้ euthyroidism ขนาดของ lingual thyroid ยุบลงบ้าง.

ในระยะต้นของ embryonic development thyroid gland ก่อตัวขึ้นที่โคนลิ้น แล้วต่อมาจึงเคลื่อนตัวมาที่คอเมื่อสัปดาห์ที่ 7 thyroglossal duct เชื่อมต่อ descending thyroid gland ที่เลื่อนลงมากับโคนลิ้น ถ้า thyroid gland ไม่เคลื่อนลงมา ก็ทำให้มี lingual thyroid
