เด็กชายอายุ 12 ปี มาด้วยอาการปวดท้อง อาเจียน และท้องโตขึ้นมานาน 3 วัน

PE : hyperpigmented macules were seen on his lips (รูป A).

คำถาม
1. CT scan of abdomen ดังในรูป B พบความผิดปกติอะไรบ้าง
2. ปัญหาของผู้ป่วยมีอะไรบ้าง
3. การวินิจฉัยโรคคืออะไร
4. จะ manage อย่างไร

1.น่าจะมี intussuseption
2 gut obstruction,hyperpitmentation on lip
3.Peutz-Jeghers Syndrome ซึ่ง leading poiunt ซึ่งเป็นสาเหตุให้เกิด intussusception
4.อาจจะ pneumatic recution และแก้ไขสาเหตุ

CT scan: proximal jejunojejunal (รูป B, arrow) and ileocolic intussusceptions.

Surgical exploration: dilatation of the small bowel and necrosis of the jejunal intussusceptum. Resection of this segment and a short ileal intussusceptum containing a large polyp was performed, followed by primary anastomosis.

Pathological evaluation: multiple hamartomatous polyps in the necrotic jejunum and an ileal polyp 3.5 cm in diameter.

การวินิจฉัยโรค: Peutz–Jeghers syndrome ซึ่งเป็น autosomal dominant disorder มีลักษณะจำเพาะคือ มีการเกิด multiple hamartomatous gastrointestinal polyps, mucocutaneous pigmented lesions ที่ริมฝีปากหรือ buccal mucosa, และเพิ่มความเสี่ยงของมะเร็งภายในและนอกกระเพาะอาหารและลำไส้
