ยาทางโรคหัวใจกับ dental manipulation: จดหมายจากอาจารย์ธาดาอาจารย์นพ.ธาดา ยิบอินซอย ได้มีหนังสือถึงคณบดีคณะทันตแพทย์เมื่อวันที่ 11 เมษายน 2551 เรื่อง ยาทางโรคหัวใจกับ dental manipulation ดิฉันได้เรียนขออนุญาตอาจารย์นำมาลงใน e-consult เพื่อเป็นประโยชน์แก่ส่วนรวม มีทั้งหมด 3 หัวข้อ คือ 1. IE prophylaxis 2. Anticoagulation 3. ASA IE prophylaxis ในยุคนี้ (ประมาณ 10 ปีที่ผ่านมา) ความเข้าใจในการป้องกันการเกิด infective endocarditis (IE) จาก dental manipulation เปลี่ยนไปมากจนผมเองแทบไม่ prophylax นอกจากรายการที่เสี่ยงมากๆ เช่น ในกลุ่ม non-biological prosthetic valves เหตุผลคงมาจาก ก. เมื่อค้นข้อมูลการศึกษาที่สนับสนุน prophylaxis IE จาก dental manipulation จะไม่พบข้อมูลนี้เลย และขณะเดียวกัน มีข้อมูลในทางตรงกันข้ามที่ไม่แรง ข. Bacteremia (transient) ใน dental manipulation versus การขบเคี้ยว แทบไม่ต่างกันเลย (คือเกือบ 100% ทั้ง 2 กรณี) ค. Expert ส่วนมากลดขอบเขตของชนิดของ manipulation และชนิดของ โรค ที่ต้อง prophylax IE จึงอยากให้ทบทวนความเข้าใจ เพื่อความสะดวกของการดูแลผู้ป่วยที่มารับบริการที่แผนกทันตกรรม ถ้าต้องการ prophylax ผมแนะว่า amoxicillin 2 g = 1 ชั่วโมงก่อน maneuver ครั้งเดียวก็พอในคนซึ่งไม่มีประวัติแพ้ยากลุ่ม penicillin Posted by : chpantip , E-mail : (chpantip@medicine.psu.ac.th) , Date : 2008-05-27 , Time : 12:07:54 , From IP : 172.29.3.68 |
ถ้าต้องการให้ IE prophylax ผมแนะว่า amoxicillin 2 กรัมประมาณ 1 ชั่วโมงก่อน manoeuvre เพียงครั้งเดียวก็พอในคนที่ไม่มีประวัติแพ้ยากลุ่ม penicillin Anticoagulation ในคนที่ได้ valve replacement และกิน anticoagulant (เรามีแต่ warfarin) คิดว่าหยุด warfarin ก่อน 5 วัน เหมาะสมที่จะลด INR (INR=international normalized ratio ซึ่งเป็น prothrombin ratio ที่ปรับตามชนิดของ thrombopalstin) ให้เข้าอยู่ในเกณฑ์ที่ไม่อันตราย (NB. อาจไม่ได้ ratio 1.00 ทุกราย) อาจ pressure dressing นานๆหน่อยก็แล้วกัน สำคัญคือ ในวันเดียวกันนั้นให้กลับไปกิน warfarin เลย อันตรายที่จะเกิดระหว่าง 10 วันที่ lost therapeutic coverage ของ anticoagulation คงไม่น่ากลัวเท่าไรในคนที่กิน warfarin มาหลายปี แต่ถ้าเขาเพิ่งเริ่มได้ warfarin อันตราย (ที่เกิดจาก under-anticoagulation) จะสูง ฉะนั้น ผมคงไม่อยากเล่นกับการหยุด warfarin ในคนที่เพิ่งได้ warfarin ภายใน 1 ปี ASA เรื่อง ASA ก็เช่นเดียวกัน ทางผมอาจ overprescribe (เพราะไม่รู้ว่าใครได้กำไรจริงๆ และกำไรอาจเพียง 1-5 คน ต่อ 1000 คน ต่อ 1 ปี) ผมไม่คิดว่าการหยุดนั้นช่วยลด bleeding tendency ในคนส่วนใหญ่ แต่ความรู้สึกว่าต้องงด ASA นี้เผยแพร่ ในคนส่วนมาก หยุดแล้วก็ไม่มีอันตรายจากโรคที่พยายามป้องกัน แต่ในคนที่จำเป็นต้องได้ ASA จริงๆ เช่น recent unstable angina การหยุดอาจเพิ่มโอกาส thrombosis โดยเฉพาะถ้าเขามีโอกาส rebound ฉะนั้น อาจยอมเสี่ยงไม่หยุดในคนกลุ่มนี้ Posted by : chpantip , E-mail : (chpantip@medicine.psu.ac.th) , Date : 2008-05-27 , Time : 16:08:18 , From IP : 172.29.3.68 |
ขอบคุณจดหมายจากอาจารย์ธาดามากๆค่ะ ที่ให้คำแนะนำในเรื่อง IE prophylaxis หนูเคยเจอ case เป็น MR หลังทำ MVR และอยากถอนฟัน แผนกทันตกรรม consult เรื่อง prophylaxis มา และหนูได้ให้ Amoxycillin 2 gm ก่อนทำ 1 ชั่วโมง แต่ได้ให้ Amoxycillin กินต่ออีก 3 วันๆละ 2 gm ค่ะ ในกรณีนี้ไม่ทราบว่าหลังถอนฟัน ATB จำเป็นหรือเปล่าคะ ขอบคุณค่ะ Posted by : Nongnham , E-mail : (Nongnham025@gmail.com) , Date : 2008-05-28 , Time : 07:08:40 , From IP : 222.123.136.200 |
อาจารย์ธาดา ตอบเรื่อง IE prophylaxis ของคุณหมอ Nongnham 1. prophylaxis ในที่นี้หมายความว่าป้องกัน ฉะนั้น ถ้าทำตามคำนิยามก็ควรให้ครั้งเดียวก่อนถอนฟัน (มีข้อมูลจากสัตว์ทดลองว่า ให้ 1 ชม. หลังถอนฟันก็ยังช่วยได้) 2. จริงแล้ว การถอนฟันทำให้เกิด bacteremia มากเท่ากับเคี้ยวหรือสีฟัน ฉะนั้น ผมถึงอยู่ในกลุ่มที่ไม่เชื่อว่า dental manipulation เป็นต้นเหตุของ IE (เชื่อว่า IE มาจากปฏิกิริยาชนิด adhesion ของแบคทีเรียเข้าไปใน endothelium ที่ผิดปกติ) 3. ในข้อมูลของผู้ป่วยที่เสนอนี้ เขามี MVR ซึ่งสำคัญมากที่ต้องจัดการเรื่องหยุด anti-coagulant ถ้าเขากินยาละลายเลือดมาหลายปีแล้ว โดยเฉพาะถ้าไม่มีประวัติของ embolism, ผมก็จะหยุด warfarin 5 วัน แล้วให้กลับไปกินวันเดียวกับที่ถอนฟัน แต่ถ้าเขาเพิ่งเริ่ม warfarin ใหม่ ๆ โดยไม่รู้ประวัติของ anti-coagulation การหยุด 5 วัน อาจจะอันตรายในด้าน systemic embolism อ.พรรณทิพย์อาจให้ความเห็นเพิ่มเติมได้ Posted by : chpantip , E-mail : (chpantip@medicine.psu.ac.th) , Date : 2008-05-28 , Time : 16:00:12 , From IP : 172.29.3.68 |
ขอเพิ่มเติมข้อมูล recommendation ของ American heart Association เรื่อง Prevention of IE 2007 ค่ะ Cardiac conditions associated with the highest risk of adverse outcome from endocarditis for which prophylaxis with dental procedures is recommended 1. Prosthetic cardiac valve 2. Previous IE 3. Congenital heart disease (CHD)* - Unrepaired cyanotic CHD, including palliative shunts and conduits - Completely repaired congenital heart defect with prosthetic material or device, whether placed by surgery or by catheter intervention, during the first 6 months after the procedure† - Repaired CHD with residual defects at the site or adjacent to the site of a prosthetic patch or prosthetic device (which inhibit endothelialization) 4. Cardiac transplantation recipients who develop cardiac valvulopathy *Except for the conditions listed above, antibiotic prophylaxis is no longer recommended for any other form of CHD. †Prophylaxis is recommended because endothelialization of prosthetic material occurs within 6 months after the procedure. Dental procedures for which endocarditis prophylaxis is recommended for patients listed above All dental procedures that involve manipulation of gingival tissue or the periapical region of teeth or perforation of the oral mucosa* *The following procedures and events do not need prophylaxis: routine anesthetic injections through noninfected tissue, taking dental radiographs, placement of removable prosthodontic or orthodontic appliances, adjustment of orthodontic appliances, placement of orthodontic brackets, shedding of deciduous teeth, and bleeding from trauma to the lips or oral mucosa.
