เรียน อาจารย์พรรณทิพย์ ผม นศพ เพิ่มพูน สวัสดิชัยกุล ผมคิดว่ารูปนี้น่าจะเป็น Strongyloides stercolaris ( ระยะ filariaform ) ครับ โดยสังเกตจากบริเวณปลายของมันที่มี notch ครับ ส่วนเรื่อง antihelminthic drug นั้น ผมคิดว่าต้องให้ครับ โดยเท่าที่ผมอ่านในหนังสือ parasitology มานั้น Strongyloides stercolaris ให้ treat ด้วย thiabendazole ครับ แต่ Dose & duration เป็นอย่างไรนั้นผมไม่ทราบครับ และเท่าที่ทราบอีกอย่างหนึ่งคือ ถ้าพบ Strongyloides stercolaris ใน stool exam ให้ระวัง gram negative bacterial infection ด้วยครับ เพราะว่า gram negative bacteria จะ infect เข้าไปใน Strongyloides stercolaris ซึ่งเมื่อ Strongyloides stercolaris ชอนไชผิวหนัง >>> เส้นเลือด >>> ไปปอด >>> หลอดลม >>> คอหอย >>> GI tract จะนำเอาเชื้อgram negative bacteria ไปด้วย อาจารย์ครับช่วยกรุณาชี้แนะผมเพิ่มเติมในเรื่องของการใช้ยาด้วยครับ ขอขอบตุณครับ ด้วยความเคารพ Posted by : k1a2i0p2p9 , E-mail : (k1a2i0p2p9@gmail.com) , Date : 2008-05-27 , Time : 20:12:53 , From IP : 172.29.22.227 |

 ขอบคุณนศพ เพิ่มพูน สวัสดิชัยกุล และ Nongnham มากค่ะ ตอบถูกทั้งคู่ค่ะ 1. รูปที่เห็น คือ ส่วนหางของ filariform larva ของ Strongyloides stercoralis ซึ่งมี notch tail และเป็น infective stage ถ้าเป็น rhabditiform larva ของ Strongyloides stercoralis หางจะแหลม (ดูรูป) การตรวจอุจจาระ wet smear ถ้าพบ larva ดิ้นๆอยู่ การหยด iodine ลงไป จะทำพยาธิหยุดดิ้นและทำให้เห็นลักษณะต่างๆได้ชัดเจน ถ้ามี filariform larva มากก็แสดงว่ามีโอกาสที่จะมี autoinfection มากขึ้นด้วย การได้รับยา corticosteroid ทำให้ rhabditiform larva เปลี่ยนเป็น filariform larva มากขึ้น ถ้าพบ Strongyloides larva ก็ต้องให้การรักษาแม้ว่าผู้ป่วยจะไม่มีอาการ เพราะพยาธินี้มี autoinfection (เพิ่มจำนวนใน host ได้) ยิ่งในกรณีที่ผู้ป่วยเกิดมีภูมิต้านทานต่ำลง เช่นจากการได้รับยา corticosteroid (แม้กระทั่งในรายที่มี acute asthmatic attack แล้วได้รับ corticosteroid 5 วัน) ก็เกิด hyperinfection syndrome ได้ สำหรับการรักษาใน uncomplicated strongyloidiasis คุณหมอ Nongnham ตอบถูกแล้วค่ะ Drug of choice and duration of treatment are:- - Albendazole 400 mg bid x 3 days then follow stool examination ที่ 2 สัปดาห์ และหลังจากนั้นที่ 3 เดือน 6 เดือน ส่วน thiabendazole ซึ่งเป็นยาที่มีประสิทธิภาพสูง แต่มีผลข้างเคียงสูงมาก ปัจจุบันบริษัทยาเลิกผลิตแล้วค่ะ Posted by : chpantip , E-mail : (chpantip@medicine.psu.ac.th) , Date : 2008-05-28 , Time : 08:39:17 , From IP : 172.29.3.68 |
