ชาย 18 ปีมีก้อนที่หลังมานาน 6 เดือน




   ชายอายุ 18 ปี มาด้วยเรื่องมีก้อนที่หลังด้านซ้ายมานาน 6 เดือน
ผู้ป่วยไม่มีโรคประจำตัวใดๆ
6 เดือนก่อน เริ่มมีก้อนที่หลังด้านซ้าย ก้อนนี้โตขึ้นช้าๆ (รูป A)
3 วันก่อน มีน้ำใสไหลออกมาจากแผลที่หน้าอกด้านซ้าย

CXR: multiple left-sided pleural nodules with a consolidation in the left lower lobe.
Computed tomography of the chest (รูป C): numerous pleural nodules encasing the left lung and a large collection of fluid in the musculature of the left back

1. จงบอกการวินิจฉัยโรคและเชื้อก่อโรคที่น่าจะเป็นมากที่สุด
2. จะ manage อย่างไรต่อไป

   TB pleura ได้ไหมครับ คงเอาน้ำหนองมาย้อมก่อนล่ะครับ

ถ้าใช่ ก็ treat ด้วย CAT 1 ไป

    actinomycosis หรือเปล่าครับ
host คนไข้ ปกติดีไหมครับ HIV or on steroid or otherimmunocompromise ?
Gram stain : น่าจะได้ Gram positive in chain and branch
modified AFB gram positive in chain and branching

ทั้งหมดนี้เข้าได้กับ empyema necessitatis ซึ่งหนองสามารถออกมาสู่ผนังอกได้ ในผู้ป่วยรายนี้ เพาะเชื้อได้จากน้ำในช่องเยื่อหุ้มปอดขึ้น Mycobacterium tuberculosis ซึ่งต่อมาพบว่าเชื้อไวต่อยารักษาวัณโรคทุกตัว HIV antibody ได้ผลลบ
ผู้ป่วยได้รับยารักษาวัณโรคสูตรมาตรฐานและ อาการดีขึ้นอย่างช้าๆ

differential diagnosis: actinomycosis, nocardiasis, non-tuberculous mycobacterial infection, fungal infections, melioidosis
