A 50 YO alcoholic man had fever and somnolence for 2 daysชายอายุ 50 ปี ดื่มสุรามานาน ญาตินำส่งรพ.เนื่องจากมีไข้และซึมลง 2 วัน ตรวจร่างกาย: T 38.5 degree Celcius, HR 110/min somnolence, not pale, no jaundice had palmar erythema, and spider nevi Abdomen: genreralized distention, no tenderness, fluid thrill and shifting dullness positive, liver span 12 cm CBC: hematocrit 33%, wbc 12000, N 82%, Band 8%, L 10%, platelet normal. UA: negative. Abdominal paracentesis: clear yellow fluid with wbc 800/cumm, N 60%, normal glucose and protein 2 g%. 1. ขอสรุปปัญหาของผู้ป่วยรายนี้ก่อนนะคะ ปัญหา - Heavy drinking - alteration of consciousness - fever with SIRS with leukocytosis with bandemia - signs of liver stigmata - ascites with infection คิดว่าผู้ป่วยรายนี้เป็น SBP ค่ะ จากประวัติและตรวจร่างกายแสดงว่าผู้ป่วยมี cirrhosis อยู่แล้วและอาจจะมี HCC ตามมาแล้ว และมี ascitis เกิดขึ้น ส่วนเรื่อง alteration of consciousness เป็นไปได้ 2 อย่างคะ คือ จาก sepsis หรือ เกิดจาก hepatic encephalopathy สำหรับเชื้อที่ทำให้เกิด SBP ในผู้ป่วยรายนี้คิดว่าเกิดจาก Escherichia coli มากที่สุดค่ะ 2. investigations Ascites culture + Cytology เพื่อช่วยในการวินิจฉัย HCC ค่ะ USG abdomen เพื่อดูสาเหตุที่ทำให้เกิด ascites 3. คิดว่าต้องให้ Antibiotic ในผู้ป่วยรายนี้ค่ะ เพราะมี systemic infection ค่ะ และ ยาที่ให้คือ Cefotaxime iv ค่ะ Posted by : nongnham , E-mail : (nongnham025@gmail.com) , Date : 2008-07-29 , Time : 12:31:44 , From IP : 117.47.183.214 |

ต้องขอโทษค่ะที่ตอบช้ามาก คุณหมอ Nongnham ตอบถูกต้องแล้วค่ะ spontaneous bacterial peritonitis ในผู้ป่วยตับแข็ง เชื้อก่อโรคจะเป็น enteric pathogen 60-69 % ที่พบบ่อยที่สุด คือ E. coli รองมาคือ Klebsiella pneumoniae, Streptococcus pneumoniae, Streptococcus อื่นๆ และ enterococci ส่วน anaerobic bacteria พบได้น้อยเพราะ ascitic fluid มีฤทธิ์ bacteriostatic ต่อ Bacteroides species และมี PaO2 สูง เห็นด้วยที่คุณหมอเลือกใช้ยา 3rd generation cephalosporin ค่ะ (จะเลือกใช้ ceftriaxone ก็ได้ค่ะ เพราะการบริหารยาจะสะดวกกว่า cefotaxime และราคาถูกกว่า) Posted by : chpantip , E-mail : (chpantip@medicine.psu.ac.th) , Date : 2008-08-09 , Time : 14:37:39 , From IP : 172.29.3.68 |
