Introduction : Crossmatching blood requests for patients are normally tested for ABO and Rh grouping, irregular antibody screening and crossmatching between patient serum and donor red cells (grouping & crossmatching). This is quite a time consuming process and all the blood tested will be hold which reduce the numbers of blood stock left for other patients. These will be unneces- sary cost in surgical patient whose procedure is rarely use blood. The typing and screening (T&S) is the test that performs ABO and Rh grouping and irregular antibody screening without crossmatch- ing. The crossmatching would be done if patients really need transfusion. T&S will be a cost saving request for this group of patients and reduce blood shortage problem.
Objective : To study the rate of transfusion in surgical patients which T&S were requested and to study the cost saving by ordering type and screen tests instead of grouping and crossmatching in Songklanagarind Hospital.
Methods : Surgical blood orders in a group of patients which T&S were requested from various departments were reviewed during January 1999-December 1999. Numbers and type of blood trans-fusion information given to patients were collected from record of the Blood Bank. The rate of trans-fusion to patients was calculated.
Results : T&S testings were requested in 1,891 patients. There were 73 cases changed from T&S to crossmatching requests prior and peri-operation. Only forty-three out of 73 patients which crossmatchings requested were given blood transfusion. There were totally 1,818 patients who were not given blood transfusion. The rate of transfusions in these studied patients was 2.3%
Discussion and conclusion : Surgical blood orders in patients who rarely need transfusion is unnecessary task of blood bank staffs and may cause blood shortage problem. T&S testing request was introduced in the Blood Bank unit to reduce unnecessary cost. The transfusion rate was 2.3%, therefore T&S testing can save the cost of crossmatching for 18,910 baht and decreased the blood shortage problem. Blood order for surgical patients who were unlikely transfused (less than 10%) should be typing and screening test.
