The incidence and factors related to acute confusional states in hospitalized elderly pateints

Acute confusional states are a common developmental risk in hospitalized elderly patients for acute illness. This study is a prospective study of the incidence and factors related to acute confusional states in hospitalized elderly patients. The samples were elderly patients who were admitted to the medical wards of Songklanakarind Hospital from January to March, 1999. The research samples, consisting of 254 elderly medical patients, were selected by purposive sampling. The instruments used in this study were : a demographic data record, the Thai Delirium Rating Scale, the laboratory results record, and the medication record. Data was collected by interviewing observation, records, and chart review. SPSS for Windows was used for data analysis. The results showed that 6.3% of the samples developed acute confusional states, and the onset of acute confusional states ranged from the 2nd - 6th day after admission. There were statistically significant differences between the samples with, and without, acute confusional states: mortality rate (p<.05) and amount of medication group used (p<.001). There were positive statistically significant relationships between length of hospital stay and amount of medications used with acute confusional states (r=.3567, p<.001, and r=.4061, p<.001, respectively). These results provide data for appropriate planning and effective intervention to prevent, reduce, and manage acute confusional states and they will also provide further indications for nursing research.
