Discharge Planning for stroke patients : Preliminary report

The Discharge Planning Project for stroke patients was implemented in the male medical ward 1, Songklanagarind Hospital, between July 11th 1997 - December 9th 1998. The project aimed at studying the effect of the discharge program on the incidence of complications, length of hospital stay and expense of hospital stay. There were 30 stroke patients admitted between May 2nd 1995 - March 10th 1997 before the project was started. The patients' age range was 45-78 years. The com-plication rate was 36.67%. The most common of complication was pneumonia (26.67%) and the second and third most common were bed sore (23.33%) and urinary tract infection (13.33%) respec-tively. The mean length of hospital stay and expense of hospital stay were 13.33 days and 21,970.50 baht. From the time of initial hospital discharge to 12 weeks after discharge, 13.33% of stroke patients were readmitted. At the time the project was implemented, there were 30 cases of stroke patients and the patients' age range was 37-89 years. The complication rate was 20%. The most common of complication was pneumonia (20%), followed by bed sore (6.67%) and urinary tract infection (3.33%). The mean length of hospital stay and expense of hospital stay were 9.47 days and 9,081.2 baht. From the time of initial of hospital discharge to 12 weeks after discharge, 10% of stroke patients were readmitted. These results showed that Discharge Planning Project could decrease the incidence of complications, length of hosptal stay, expense of hospital stay and readmission rate.
